Waxing Consent Form​

Consent and Release:

I understand that waxing is a cosmetic procedure that involves the removal of hair from the skin's surface. I have been informed about the potential risks and benefits associated with waxing, including but not limited to redness, irritation, ingrown hairs, and the possibility of skin tearing or bruising. I have disclosed all medical conditions, allergies, medications, and recent skin treatments to the best of my knowledge.

I acknowledge that I have been provided with an opportunity to ask questions regarding the waxing process, aftercare, and any concerns I may have. The esthetician has explained the procedure and provided recommendations for aftercare.

I understand that the esthetician will do their best to ensure a comfortable and safe waxing experience, but results may vary depending on my individual skin type and hair growth patterns.

I hereby release and discharge the esthetician and the establishment from any and all claims, liabilities, and damages arising from or in connection with the waxing services provided.

Photography Release:

I give permission for photographs to be taken before and after the waxing service. These photos will be used for documentation and may be used for educational or promotional purposes, while ensuring my anonymity.
